Foto Copy Ijazah SMA/SMK/Sederajat, atau Surat Keterangan yang sudah dilegalisir 2 lembar
Pas photo terbaru berwarna ukuran 2x3, 3x4 masing-masing 4 lembar
Semua persyaratan dimasukkan dalam map warna kuning

WAKTU PENDAFTARAN

GELOMBANG I: April s/d Mei
GELOMBANG II: Juni s/d Juli
GELOMBANG III: 8 Agustus

TEMPAT PENDAFATARAN

Pendaftaran dibuka setiap hari kerja:
